Individualized nursing of a patient with bronchobiliary fistula after hepatectomy for liver cancer (1例肝癌切除术后胆管支气管瘘患者的个体化护理体会)

Abstract

Bronchobiliary fistula is a rare clinical pathology, which severely affects patient’s quality of life. This paper reported individualized nursing management for a patient with bronchobiliary fistula after hepatectomy for liver cancer. Key issues of nursing included: Strictly implementation of anti-infection treatment and prevention of secondary infections and severe complications; maintenance of the drainage tube and medication care; comprehensive nutrition support; appropriate dressings changes and wound care; effective pain control; individualized psychological intervention on negative emotion. The patient was discharged after healing of the bronchobiliary fistula and relieving of bile leaks, and patient was in good condition after 6-month follow-up. (胆管支气管瘘是一种临床少见的病理情况, 患者特征性临床表现为咳胆汁样痰, 严重影响生活质量。本文报告1例肝癌切除术后胆管支气管瘘患者的护理经验。护理要点: 严格落实抗感染治疗, 防止继发感染及严重并发症; 做好各种引流管及特殊用药护理; 给予全面的营养支持; 采用合适的敷料换药, 加强伤口护理; 有效控制疼痛; 针对患者出现的负性情绪, 实施个性化心理干预。经过3个月积极治疗和精心护理, 患者胆管支气管瘘愈合, 腹腔胆漏明显改善后出院, 随访6个月, 生存质量良好。)
