Patients with lateral foot pain at the base of the 5th metatarsal may present for a variety of reasons. A common cause is due to an un-united 5th metatarsal avulsion fracture. Although commonly treated with conservative care, these patients periodically need late-stage surgery due to painful nonunion. The small nature of the avulsion fragment makes nonunion repair challenging and therefore, excision may be the best option. Excision of the fracture fragment and peroneal tendon reconstruction is similar to a Kidner procedure, however there is no current medical literature to guide the surgeon with patient selection or surgical technique. This report will focus on patient selection including expanded indications, surgical pearls, and postoperative care.
