The presence of a non-compacted left ventricle is a novel and uncommon myocardial disease. It can be classified as single or associated depending on the presence or not of other cardiac or extracardiac anomalies. Both are characterized by the presence of a large myocardial trabecular layer with several deep recesses which communicate directly with the ventricular cavity and a proportion of non-compacted left ventricle/compacted left ventricle which is higher than two at the end of systole. Among the most commonly described associated anomalies are the neuromuscular illnesses. In the present article a case of a non-compacted left ventricle associated to a neurofibromatosis type I is presented and a brief and up-dated review of this novel heart disease is made – a disease which has increased its worldwide importance in Cardiology.
