Aim of the study. The purpose of this article was to review current confirmation on opinion, risk assessment, and prevention, compare the implications of fixed braces and invisible aligners, and evaluate possible implications on periodontal status. Material and methods. A manual literature search was performed using the PubMed, Wolters Kluwer, and Cochrane databases. For evaluation keywords such as “invisible aligners” and “periodontal health” in different combinations were used during the study methodology. Results. The comparison at the baseline (T0) showed increased bleeding on probing BOP in the fixed orthodontic group, supporting that when no adequate information is provided to the patients, poorer oral hygiene can be observed in patients wearing multibracket appliances. The periodontal parameters, like plaque index PI showed beat values in T2 compared to T0 and T1 in the fixed orthodontic appliance group, while the clear aligners group showed an increase in PI value in T2 compared to T0. Conclusions. The groups treated with Clear aligner appear to be facilitated in hygiene procedures and therefore patients with periodontal problems are more directed to be treated with clear aligners in order to avoid further risks to the periodontium.
