Although thrombolysis improves the prognosis of the patients with acute myocardial infarction, an increase of mortality on the first day has been described. This paper, which is based on a recently finished study where 2 915 patients with this affection were treated with Recombinant Streptokinase, is aimed at knowing retrospectively the distribution of deaths in the first 7 days after thrombolysis, the relationship between letality on the first day and the causes of death during the following days and their distribution in time. The number of deaths was higher in the first 2 days (27,8 and 21,5%), whereas letality on the fisrt day was significantly higher in patients over 60 and women. The fail of the pump was the most frequent cause of death followed by heart rupture and arrhytmias, which ocurred mainly in the first 3 days after infarction.
