The article discusses mechanisms of effect of Actovegin in the context of studying its effects on pre-clinical and clinical levels in the treatment of coronary artery disease. Actovegin, obtained by ultrafiltration of calves’ blood, includes more than 200 biological substances. The results of preclinical and clinical studies have shown that Actovegin improves metabolic balance by increasing glucose uptake and oxygen consumption in ischemia, which is the basis of the antioxidant and anti-apoptotic cardioprotection. Multi-factorial and multi-component nature of Actovegin determine its pleiotropic cardioprotective mechanism and clinical efficacy. It was shown by the results of clinical experience treating 46 patients: Actovegin helps to reduce the incidence of reperfusion syndrome in thrombolytic therapy of acute myocardial infarction by almost half, has antiarrythmic effect.
