Abdominal masses in the pediatric population are usually retroperitoneal in location and 50% are renal in origin. In less than six months of age, the most common renal tumor is mesoblastic nephrons and the frequency of t Wilms' tumor increases after that. Neuroblastoma is the most common retroperitoneal tumor in those less than two years of age and commonly adrenal in origin. Its diagnosis is difficult and challenging, since both Wilms' tumor and intrarenal neuroblastoma may present with the similar clinical and radiological findings. It is crucial to differentiate between them given the different treatment modalities and clinical outcomes.
