Algunos aspectos en el cuidado crítico de la paciente obstétrica Critical care in the obstetric patient
Abstract
Las pacientes embarazadas críticamente enfermas representan un reto para el equipo de salud debido a los cambios fisiológicos propios de este estado, a que se trata de dos pacientes (madre y feto) con fisiologías distintas, a los cambios anatómicos en la madre y, por último, a la presencia de patologías exclusivas del embarazo como la preeclampsia, el abruptio y el embolismo de líquido amniótico, entre otras. En este escrito se hace una revisión breve de las adaptaciones fisiológicas del embarazo y la fisiología fetal, las bases del monitoreo invasivo y algunas consideraciones en torno a situaciones específicas en estas pacientes; no se abordará el manejo de las patologías propias del embarazo, porque desbordan los objetivos de esta revisión.Critically sick pregnant patients represent a challenge for the health team due to the complex physiological changes of the associated conditions, because there are two patients (mother and fetus) with different physiologies, because of the anatomical changes in the mother, and because of the presence of exclusive pathologies of pregnancy (such as pre-eclampsia, abruptio placentae and amniotic fluid embolism, among others). In this review we make a brief account of the basic physiological maternal adaptations for pregnancy, a brief review of fetal physiology, the bases of the invasive monitoring, and some considerations about specific situations with critically ill patients. We will not review the management of specific pathologies of pregnancy.
