Preventive antibioticotherapy and local sanation of the pathological foci in the treatment of infected pancreonecrosis and its complications

Objective. To analyze and estimate the application efficacy of preparation grandazol for preventive antibioticotherapy in acute pancreatitis, and of antiseptic decasan - for local sanation of purulent foci in pancreonecrosis and its complications. Маterials and methods. The results of application of domestic preparation grandazol for preventive antibioticotherapy in 10 patients, suffering an acute destructive pancreatitis, and of antiseptic solution of decasan - in 23 patients, suffering focal complications of an acute pancreatitis, were analyzed. Results. In 8 (80%) patients an early application of grandazol for preventive antibioticotherapy was effective, in them the destruction foci infectioning was not observed. In 2 (20%) patients the pancreatitis foci infectioning have had occurred. In 23 patients the postnecrotic accumulations sanataion was performed, using decasan in a 0.02% solution, in 19 (82.6%) of them a complete sanation of purulent foci was achieved due to puncture and drainage with subsequent outflowing, using solution of decasan. In 4 (17.4%) patients, in whom in postnecrotic accumulations the tissue component (sequesters) prevailed, the operative interventions (necrsequestrectomy) and adequate drainage of residual cavity were conducted. Conclusion. Antibioticotherapy with applying of the fluorochynolones group antibiotics (grandazol) and adequate sanation of pathological foci in destructive pancreatitis in conjunction with local usage of decasan 0.02% are purposeful and improve the AP results of treatment.
