Ergotism is a rare clinical condition characterized by generalized vasoconstriction produced by the administration of Ergot-derived drugs. Primarily mediated by drug interactions, including antibiotics, oral contraceptives, and antivirals. We present the case of a 28-year-old patient with human immunodeficiency virus infection under antiretroviral management with protease inhibitors who concomitantly consumed ergotamine with which he developed intense pain, coldness, paresthesias and cyanosis of the lower limbs associated with a Ulcerated lesion on the dorsum of the right foot, episode of vasospasm secondary to ergotism was considered, vasculitis is relevant within the differential. I present improvement after the suspension of the medications that were related and the use of a vasodi-lator. The purpose of the case is to emphasize a serious drug interaction such as ergotamine plus protease inhibitors.
