Lumbar spinal stenosis is widespread pathology. It is found at almost 8% of population. Modern classifications of lumbar spinal stenosis and it’s etiologic factors are given. Pathophysiological mechanisms of stenosis are caused by three groups of factors: epidural pressure increasing, aseptic inflammation and ischemia. That causes characteristic patient’s complaints — back pain, neurogenic claudication, radiculopathy.Spinal stenosis treatment, as a rule, is surgical, conservative treatment in ineffective. In our days such types of surgical interventions are performed at lumbar spinal stenosis: decompressive laminectomy, fusion and dynamic interspinous devices setting. Each of these methods has it’s own advantages and failings. Efficacy of this operations, their pathophysiological ground and features of application at stenosis, combined with instability, are described in details.
