It is quite difficult and technically challenging to achieve correct occlusion post trauma in case of high condylar extra capsular fractures. The reason for not achieving correct occlusion is a difference in the ramal height. In cases of closed reduction, the ramal height cannot be equated. The technique described in this article is to gain post ramal height by performing a posterior border reverse ''L'' Osteotomy and condylar head as a free graft for correction of ramal height. This will help us to achieve equal ramal heights and a corrected occlusion , midline with good mouth opening.
