Application of PDCA cycle in early rehabilitation nursing of patients acute ischemic stroke (PDCA循环在缺血性脑卒中急性期患者早期康复护理中的应用)
Abstract
Objective To explore the effect of PDCA cycle in the early rehabilitation nursing of patients with acute ischemic stroke. Methods Totally 65 patients with acute ischemic stroke received from February to October 2019 were included as the control group, and 60 patients with acute ischemic stroke received from November 2019 to July 2020 were included as the study group. The control group was given routine care. In the study group, the PDCA cycle was established and early rehabilitation nursing interventions according to the four stages of Plan- Do- Check-Action were carried out. The neurological impairment of patients was measured by China Stroke Scale (CSS). The activity of daily living of patients was evaluated by Brathel Index (BI). The hemiplegic motor function of patients was evaluated by Brunnstrom Scale. Results After implementation of nursing interventions, the CSS score of the study group was lower than that of the control group(P<0. 01), and the BI index of the study group was higher than that of the control group(P<0. 01). The subscale of upper limbs, hands, lower limbs and total score of the Brunnstrom scale of the study group were higher than those of the control group(P<0. 01). Conclusion In early rehabilitation nursing of patients with acute ischemic stroke, PDCA cycle is an effective tool to improve the nursing quality, resulting in improvement of neurological function, motor function and quality of daily living. (目的 探讨PDCA循环在缺血性脑卒中急性期患者早期康复护理中的应用效果。方法 神经内科2019年2月—10月收治的缺血性脑卒中急性期患者65例设为对照组, 2019年 11月—2020年7月收治的缺血性脑卒中急性期患者60例设为研究组。对照组实施常规护理, 研究组在对照组基础上, 成立PDCA循环团队, 按照计划(Plan)-实施(Do)-检查(Check)-行动(Action)四个阶段实施早期康复护理。采用脑卒中临床神经功能缺损评分标准(CSS)评价患者神经功能缺损情况、采用Brathel指数(BI)评估日常生活功能, 采用Brunnstrom量表评价偏瘫运动功能。结果 实施干预后, 研究组CSS评分低于对照组, BI指数高于对照组, 差异有统计学意义(P<0. 01)。实施干预后, 研究组Brunnstrom量表的上肢、手、下肢及总分均高于对照组, 差异有统计学意义(P<0. 01)。结论 运用PDCA循环提升缺血性脑卒中急性期患者早期康复护理质量, 能减少患者神经功能缺损程度, 提升患者运动功能及日常生活功能。)
