A qualitative study on the experience of nurses in the health education for patients with ulcerative colitis based on the Ottawa decision support framework (基于渥太华决策支持框架的护士参与溃疡性结肠炎患者健康教育真实体验的质性研究)

Objective To investigate the experience of nurses in the health education for patients with ulcerative colitis, and provide a basis for optimizing health education programs. Methods This study was conducted based on the Ottawa decision support framework. Through purposive sampling, semi-structured interviews were conducted with 11 nurses from the Department of Gastroenterology. The Colaizzi's 7-step analysis method was used to analyze, summarize and refine the themes of the interview data. Results The experience of nurses can be summarized into three main themes and six sub themes: Cognitive limitation(role ambiguity, priori knowledge and contingency on authority); Imbalance between supply and demand(shortage of specialized nurses, lack of systematic planning); Ineffective evaluation(Insufficient quantitative tools, unclear indicators). Conclusion Nurses face problems such as role ambiguity, insufficient resources and difficulty in effect assessment in health education for patients with ulcerative colitis. It is recommended that hospital managers should strengthen the training of specialist nurses, develop standardized health education pathways, and construct a quantitative assessment system to improve the quality of health education. (目的 探讨护士参与溃疡性结肠炎患者健康教育的体验, 为优化健康教育方案提供依据。方法 基于渥太华决策支持框架, 采用目的抽样法, 选取2024年4月—6月天津市某三级甲等医院消化科的11名护士进行半结构化访谈, 运用Colaizzi七步分析法对访谈资料进行编码与主题提炼。结果 共提炼出3个主题, 6个亚主题: ①认知局限(角色定位模糊、先验性权威依赖); ②供需失衡(专科护士短缺、系统规划缺失); ③效果评估缺位(量化工具不足、指标不明确)。结论 护士在溃疡性结肠炎患者健康教育中面临角色认知偏差、资源不足及效果评估困难等问题。建议医疗机构加强专科护士培养, 制定标准化健康教育路径, 并构建量化评估体系, 以提升健康教育质量。)
