We present a case of successful transplantation of a lacerated cadaveric kidney. There have been very few case reports where a lacerated kidney has been transplanted. We accepted a waiver kidney which had a laceration near the lower pole, which was rejected patients in the priority list. This presented a big surgical and medical hurdle to us. To prevent bleeding after re-anastomosis, the kidney was repaired with interrupted prolene sutures and a polypropylene mesh was sutured to form a sling around the lower pole. Blood pressure was maintained in the low normal range to prevent the expansion of hematoma. The patient attained normal serum creatinine. Thus, in a country with severe organ shortages and organs that are marginal or that may be discarded can be utilized to achieve excellent results with appropriate measures and surgical technique.
