A 17-year-old girl presented to our institution with abdominal and pelvic pain found to be due to extrinsic compression of the left renal vein, causing a saccular aneurysm and renal vein dilation with varices consistent with nutcracker syndrome. She was managed with an open aneurysm resection and transposition of the left renal vein. Keywords: Nutcracker syndrome, Renal vein aneurysm, Vascular anomalies, Renal vein transposition
