Aim: to understand the perception of professionals and users about the referral and the assistance to the person with mental disorder of a post-conflict psychosocial substitute services hospitalization. Method: qualitative research, held in a Center of Psychosocial Attention and in an Psychosocial inpatient Unit from June to September 2016. Data were collected through semi-structured interviews with users and health professionals of both services and examined from the thematic analysis. Results: challenges and possibilities in the referral of the user in hospital pre-discharge and possible strategies and devices to delineate the assistance to the user in the substitutive service were evidenced. Conclusion: in order to "do" mental health care, it is necessary to plan and relate theory to practice in order to reflect a qualified care to the user.
