A Drop of Trouble: Uncovering the Systemic Adverse Effects of Topical Pilocarpine

Topical ophthalmic medications are frequently prescribed by eye care professionals, including optometrists and nurses with specialized training. However, these medications can lead to systemic side effects, particularly in elderly patients who may have multiple underlying health issues. We report the case of a 70-year-old male who underwent YAG laser peripheral iridotomy in both eyes. Shortly after the procedure, he exhibited symptoms consistent with a myocardial infarction. In view of thorough investigations showing normal results, this incident suggests that the systemic absorption of pilocarpine eye drops may have contributed to his clinical presentation.
