Some aspects of diagnosing the nervous system disturbance in patients with renal pathology
Abstract
The case report of female patient, aged 56, who suffered from glomerulonephritis for 35 years, is presented. The patient underwent a comprehensive examination, including a detailed clinical and neurological check-up, the follow-up neurological status investigation, neuropsychological examination (by means of the State-Trait Anxiety Inventory), laboratory analysis (electrolyte balance, nitrogen metabolism, cerebral and renal tissue affection autoimmune markers parameters measurement) and instrumental examination (renal ultrasonography, the follow-up brain magnetic resonance imaging). It is detected the signs of the simultaneous destruction of the central, peripheral and autonomic nervous system structures, their severity correlated with the core process activity in kidneys and was most likely of autoimmune origin. The large number of pathogenic mechanisms affecting the central, peripheral and autonomic nervous system under renal pathology, the insufficient information on their mutually conditioned impacts require further detailed comprehensive study of clinical, neurological, neuropsychological characteristics, the immune status indices, including studying the markers of autoimmune renal parenchyma and the central and peripheral nervous system structures injury. This equates to specify the pathogenetic and prognostic significance of the nervous system disturbance degree in patients with renal pathology, as well as to assess renal pathology prognostic significance for neurological and neuropsychological disorders onset and progression.
