Abstract Postpartum hemorrhage is a rare but life threatening occurrence, as is use of the massive transfusion protocol and transfusion reactions. The identification and treatment of these pathologies are emergent and potentially life-saving. This is a high-fidelity simulation case regarding a patient with postpartum hemorrhage and resulting hemorrhagic shock requiring massive transfusion. This is complicated by a transfusion reaction that must be recognized and rectified. Additional complicating factors can be added for more advanced learners as well. This high-fidelity simulation case has been run for emergency medicine residents, as well as for practicing physicians in emergency medicine and family medicine, and obstetrics residents. This case was very well received and reviewed by all participants.
