Methadone induced hypoglycemia, improved on dose adjustment

There exists reports in the literature that connects opioids with hypoglycemia, primarily methadone. We present a case of new onset hypoglycemia in a patient with end stage renal disease who was chronically on methadone. After other causes have been ruled out including insulinoma, methadone induced hypoglycemia was diagnosed. Making adjustments in doses of methadone showed improvement in hypoglycemia episodes. A brief review of literature about Methadone induced hypoglycemia which can probably be a drug class effect will be discussed here. Dose adjustments should be considered in such cases, especially in patients with comorbidities like renal disease.
