ECONOMIC ASSESSMENT OF THE LOSES FROM THE STATE BUDGET OF UKRAINE AND PERSONAL EXPENSES OF THE PATIENTS UNDERGOING HARTMAN’S PROCEDURE

In economically developed countries the health system operates under strict rules of health insurance. Typically, the provision of certain medical services takes into account the economic feasibility of the necessary surgery or any other medical manipulations, calculate potential losses for both the patient and the state. The aim of the study – to examine and economically justify the additional costs of the state and personal expenses of patients for whom Hartmann operation was performed and to compare these indicators of economic losses with patients operated by the method developed in our clinic. Materials and Methods. According to the Ministry of Health of Ukraine in 2015 the number of first recognized disabled of group II (diseases of the digestive system) was – 3423 people, including – 3219 persons in working age or 94 % [2]. Incomes of population – important resulting indicator, which characterizes the total amount of cash and natural (in monetary evaluation) income of the citizens per year. According to the State Statistics Service of Ukraine in 2015 the overall average income per employee amounted to 10362 UAH. Results and Discussion. Provided the disabiled person of the second group officially did not work, he did not receive salary, but only received social assistance and other social transfers, in this situation, the loss of the state budget in 2015 for such persons up to 5958.3 UAH per month or 11.7 % more than for disabled person who work. In addition, disabled person of the second group incurred personal additional costs associated with the acquisition of ostomy pouching system and stoma care products. To provide one patient of stoma care products necessary 10300 UAH /1 year. Conclusions. Patients who underwent Hartmann operation require prolonged social adaptation and psychological assistance to adapt to the new conditions of existence, and is also necessary to involve additional costs of funds from the state budget and personal savings, increase social assistance from the state, etc.
