Bilateral anterior lenticonus in Alport syndrome

Anterior lenticonus is a structural anomaly of the crystalline lens characterized by a localized conical or spherical protrusion centered about the pole of the anterior lens surface. A 23-year-old female presented with a gradual, progressive, painless decrease in vision of both eyes associated with hearing loss from childhood. The anterior segment examination showed a conical protrusion of the lens in the pupillary axis. Fundus showed pigmentary changes and a dull foveolar reflex-lozenge sign. Clear lens extraction was done for both eyes. The central protruded fragile lens capsule was meticulously handled during surgery to create well centered circumcorneal capsulorhexis using micro forceps.
