The results of pilonidal disease treatment depending on the method of radical surgery

Background. Of all coloproctological diseases, the pilonidal sinus (also known as epithelial coccygeal passage) ranks fourth and its prevalence is 20% of all visits to the doctor with coloproctological diseases. Due to the lack of unified approaches in studying the etiopathogenesis of this pathology, a wide variety of surgical treatment methods of the chronic form of this disease emerged. Materials and methods. The results of treatment of 502 patients with chronic pilonidal disease were analyzed. Time needed for healing of postoperative wounds and relapses of diseases were compared depending on the chosen method of complex surgical treatment. Results. The number of relapses of the disease in patients, regardless to the method of radical surgery, did not differ significantly (p > 0.05) with a statistically significant difference in the healing time of the postoperative defect between patients who underwent closure of the surgical wound and marsupilization (p = 0.001), as well as between patients who underwent median closure of the wound and patients with open management of the surgical defect (p ≤ 0.05). Conclusions. The revealed data shows the prospects of using the methods of radical treatment of pilonidal disease with median suturing of the surgical skin defect, provided that postoperative complications are prevented and the failure of skin sutures or their forced removal is prevented.
