Key Clinical Message Cholestatic hepatitis is a rare complication of Epstein–Barr virus infection and is often self‐limiting. Systemic signs, including viral prodrome and lymphadenopathy, can provide crucial diagnostic cues and avoid unnecessary invasive investigations or aggressive medical therapy unless indicated.
