Nursing Care Applied with Transitional Care Model: A Case Report of an Elderly Patient with Open Heart Surgery

Abstract

Although advances in medical technology and surgery have increased the success rate in open heart surgery, it has been reported that elderly individuals face physical, psychological and social problems in the postoperative period. Heart surgery is an important transition period in the life of the patient and his family, and this is one of the times when they need the most support of health professionals. It is seen that various care models are used and recommended in the literature in order to meet the needs of patient and family during this transition period. One of them is the Transitional Care Model. In this article; nursing care management of a 71-year-old male patient who underwent open heart surgery according to the Transitional Care Model was considered and aimed to set an example for the use of this model.
