Unexplained dyspnea with recurrent right-side pleural effusion

Exudative pleural effusions remain undiagnosed in about 20–30% of patients in spite of routine fluid examination and closed pleural biopsy. An undiagnosed pleural effusion results in patient anxiety, several physician consults, and disruption of patient’s life. On long-term follow-up, the majority of lymphocyte predominant exudative effusions turn out to be malignant. But here, we present a case with unexplained dyspnea on exertion and recurrent right-side pleural effusion. Right cardiac catheterization revealed the diagnosis. The definite treatment was surgical.
