Relationship between Quality of Life and Nutritional Status on Chronic Kidney Disease Patients with Dialysis Type

Introduction. Dialysis is a common procedure routinely used in Indonesia which consist of hemodialysis (HD) and continuous ambulatory peritoneal dialysis (CAPD). Increasing number of CKD patients lead to higher dialysis requirement. Malnutrition, adverse effect of therapy, psychological stress, and comorbid conditions may affect quality of life (QoL) in CKD patients. We aimed to investigate comparison of QoL and nutritional status between CKD patients treated with HD and CAPD. Methods. This was a cross-sectional study with total subjects of 60 CKD patient (30 HD and 30 CAPD) who met inclusion criteria. This study was conducted in August-September 2019 in Dr. Moewardi General Hospital, Surakarta. Quality of Life (QoL) and nutritional status was assessed using validated kidney disease quality of life short form-36TM (KDQOL-36TM) and subjective global assesment (SGA) form. Data was analyzed statistically with independent T-test and Chi square, p value < 0.05 considered significant. Results. Mean of age in HD group was 49.23 (SD 12.25) years and CAPD group was 41.13 (SD 12.10) years. Result of QoL showed that CAPD patients had better physical health (p = 0.003) and mental health (p = 0.047) compared to HD patients. Clinical manifestations of CKD were also more severe in HD patients (p = 0.037). Moreover, effect of kidney disease was further reduced the QoL of patients undergoing HD (p = 0.018). Meanwhile, the burden of kidney disease was not significantly difference between the two groups. Nutritional status analysis showed that CAPD patients had better nutritional status compared to HD patients (p = 0.016). Multivariate analysis resulted that mental health (p = 0.016) and physical health (p = 0.003) were the most significant component in QoL and nutritional status between HD and CAPD patients. Conclusion. Quality of life (QoL) and nutritional status were better in CAPD patients compared to HD patients. However, further studies with larger number of subjects and using different questionnaires are needed.
