Treating necrotizing diabetic infections can lead to significant loss of skin and soft tissue, making primary closure ill-advised. Managing large diabetic wounds is challenging and more so in community hospitals with limited resources and specialists.1 Vessel loop gradual closure has been described for the successful closure of expansive wounds secondary to compartment syndrome fasciotomies and other noninfected expansive wounds. In this report we describe a novel use of vessel loop gradual closure to successfully treat infected, diabetic wounds.
