Dynamics of ultrasound and electrophysiological parameters in patients with epileptic clinical type of cerebral АVМ before and after surgical treatment
Abstract
Diagnostic investigations were performed in 28 patients (4 children and 24 adults) with cerebral arteriovenous malformation (АVМ) of parieto-temporal, parieto-occipital and frontal localization. Instrumental examinations were performed before the operation and on each stage of surgical treatment. The following types of neurosurgical treatment were applied: operations with repeated endovascular embolisation (in 19 patients with AVM without cerebral haematoma); direct surgery without endovascular technique; direct operation followed by endovascular embolisation; endovascular interventions alone (in 9 patients with AVM, complicated by cerebral haematoma).The diagnostic value of ТCDS-criteria in patients with AVM has been estimated. The blood velocity (VPS) in feeding artery did not increase in almost 50% of patients with small АVМ. Large АVМ, which usually fed from different vascular sources, accompanied by increase of VPS in all intracranial arteries, decrease of РІ, RI, CVR values, impairment of the blood flow autoregulation; significant changes on EEG. Use of the digital EEG in diagnostic of patients with cerebral AVM state is indicated for the purpose of epileptiform syndrome effective control during the treatment.
