Surgery in any patient has risks and benefits and minimally invasive routes are typically preferred, however these techniques may be challenging in obese patients these patients are already typically considered to have increased intraoperative and postoperative risks. The following case report describes a patient with BMI >70 who presented for surgical sterilization. Robotic vaginal natural orifice transluminal endoscopic surgery (robotic vNOTES) was used to perform bilateral salpingectomy. This technique combines the benefits of robotic surgery for improved visualization and fine surgical movement was as well as the vaginal, minimally invasive route. Various techniques were used for successful completion of the surgery and are described in detail in the following report.
