Case Report of A Gingival Presentation Unveiling A Potentially Fatal Disease: A Dual Rarity

Granulomatosis with polyangiitis (GPA) is a rare disease affecting the small- to medium-sized vessels of various organs. The respiratory tract, lungs, and kidneys are usually affected. The gingival involvement in GPA is rare and features a classical presentation called strawberry gingivitis. GPA is associated with subepithelial neutrophilic microabscesses, leukocytoclastosis, vasculitis, geographic fibrinoid necrosis, and palisading macrophages. The elevated levels of cytoplasmic-antineutrophilic cytoplasmic antibody (c-ANCA) differentiate it from other vasculitis. A rare case of GPA who presented with strawberry gingivitis with further systemic involvement and elevated c-ANCA, confirming our diagnosis has been discussed with an emphasis on the clinical presentation, histopathological features, and management protocols followed.
