Abstract This PowerPoint presentation provides learners with information regarding older adults' capacity to live safely and independently in the community using the capacity, assessment, and treatment (CAT) model. We also provide an implementation guide for those wishing to adapt this material for their current setting. This resource underwent a successful 2-year pilot test in the geriatrics clinic of a local community hospital with geriatrics fellows and received favorable reception. Some of the lessons learned during the pilot implementation study include the importance of gaining support from your institution, clearly defining roles of team members, encouraging a sense of ownership among team members, and careful monitoring of outcomes and steps taken. Given its comprehensive nature, this resource may appear to be daunting at first glance. However, the components can be easily tailored to match the setting, staffing, and resources available to those implementing the intervention.
