An osteosarcoma is a malignant tumour of mesenchymal origin, typically occurring in the metaphysis of long bones. They are rarely found in the head and neck region. The commonest site in the head and neck region is in mandible and maxilla. Primary osteosarcomas of the skull and skull base has been reported in literature. However, an osteosarcoma presenting as a frontal sinus mucocele has yet to be reported. We present a case of a primary osteosarcoma of the right frontal sinus masquerading as a recurrent right large frontal mucocele and discuss the diagnostic difficulties faced.
