Injuries to the extrahepatic bile ducts are a group of extremely rare conditions due to the anatomic position of these organs. Generally, the diagnosis of traumatic lesions of the gallbladder and bile ducts is difficult to be established before carrying out a laparotomy. A case of injury to the main bile duct due to a blunt abdominal trauma –an iron bar fell upon him– is reported. Several radiological studies were conducted. They showed a middle-level bilateral pleural effusion and free peritoneal fluid in all areas with dilated thin handles. It was decided to surgically explore the patient with the Kocher maneuver.
