ACOUSTIC MORPHOLOGY OF STEPS IN REPARATIVE PROCESSES AFTER MICRO-INVASIVE NON-PENETRATING DEEP SCLERECTOMY WITH IMPLANTS OF VARIOUS TYPES
Abstract
Purpose. To analyze results of UBM examinations of surgical pathways of aqueous humor (AH) outflow in the drainage zone formed after glaucoma surgery with implants.Material and methods. The follow-up study included 110 patients (120 eyes) aged from 42 to 76 years with the primary open angle glaucoma (POAG) who underwent the micro-invasive non-penetrating deep sclerectomy (MNPDS) using the Russian-made hydro-gel implant and the Healaflow substance drainage (Anteis, Switzerland). The UBM was performed to determine the qualitative and quantitative parameters of structures in the surgically formed AH outflow pathways.Results. During the entire follow-up, all patients had a stable IOP normalization which did not exceed 21mmHg without any additional instillations. The UBM in dynamics revealed certain regularities of condition in forming drainage pathways at different periods after the MNPDS with implants. The research of areas of the MNPDS surgery by the method of ultrasound biomicroscopy showed that the Healaflow substance and the hydro-gel implant promoted a maintenance of structures in the drainage system created by the surgery.Conclusion. The periods from 1 to 3 months are characterized by an absence of inclusions in the cavities of drainage pathways. Beginning from the third month post-operatively a tendency is noted for a stabilization of linear and volume parameters of outflow pathways that provides indirect evidence of completion in reparative processes. In the long-term follow-up (one year) there is a trend for diminishing of linear and volume parameters of outflow pathways that is especially pronounced in patients with faradvanced glaucoma. At the end of the follow-up period statistically significant differences between groups in height of intrascleral cavity (р>0.05) are not detected.
