Objective: Report a case of leprosy in the course of immunobiological therapy in patients with rheumatoid arthritis. Case description: Female patient, 59 years, with rheumatoid arthritis, in use of etanercept, with disseminated erythematous rash associated with nodules in upper and lower limbs. Physical examination: erythematous-infiltrative plates in the face, nodules and erythematous plaques on the lower limbs and elbows. Skin biopsy: perivascular lymphocytic infiltration and perineural with granuloma formation. We conclude by Leprosy, initiated steroid therapy and antimicrobial therapy, improving the skin condition. Comments: Was identify only one publication of Hasen disease after the use of etamecepte but in psoriatic arthritis. The mechanism proposed for the majority frequency of granulomatosis diseases with the use of anti-TNF would be a stimulus for macrophages apoptosis, interferon-gamma inhibition and inhibition of macrophages receptors
