Study of the prognostic factors of pregnancy in infertile couples managed outside in vitro fertilization by intra-cytoplasmic sperm injection in western Algeria
Abstract
Introduction - Medical assistance procreation allows the treatment of infertile couples by numerous techniques: intra-cytoplasmic sperm injection, in vitro fertilization and intrauterine insemination. However, the treatment of infertile couples is not limited to medical assistance to procreation. The proportion of natural pregnancies is far from negligible with simpler means. This method of obtaining pregnancy depends on the prognostic factors of infertility. These factors are represented by the age of woman and man, passive smoking, duration and type of the infertility, as well as the causes of the infertility. This study was conducted to identify the prognostic factors of pregnancy in infertile couples managed outside of in vitro fertilization or intra-cytoplasmic sperm injection. Materials and methods - This prospective mono-centric study was carried out from January 2009 to December 2012. We performed statistical comparisons between two groups: a group with pregnancy (n = 147) and a group that failed to conceive (n = 458). A multivariate analysis by binary logistic regression was performed to identify prognostic factors. Results - The study collected 605 couples. The number of pregnancies obtained was 147 (24.3%). The multivariate analysis retained the following prognostic factors: the age of woman over 35 and that of man over 45, passive smoking in woman, duration and primary infertility, as well as severe oligo-astheno-teratospermia. Conclusion - Factors contributing to pregnancy are intimately related to the prognostic factors of infertility. The age of the woman and the man, the type, the duration and the cause of the infertility, are determining of the percentage of success and the delay of occurrence of the pregnancy apart from the in vitro fertilization or intra-cytoplasmic injection of spermatozoa.
