Diverticulitis is a common complication of colonic diverticulosis and can lead to a range of serious issues, including perforation. Incisional hernias, often resulting from previous abdominal surgeries, usually contain omentum or bowel loops. However, it’s uncommon for both conditions to present together, especially in the same anatomical location. We report a rare case of a 64-year-old male who presented with sudden abdominal pain, nausea, and vomiting. Examination revealed a painful, erythematous para-umbilical hernia. CT imaging showed an incisional hernia containing a perforated segment of sigmoid colon with diverticulitis. The patient underwent emergency surgery with sigmoid resection, primary anastomosis, and hernia repair. His recovery was smooth, and he was discharged in good condition.
