The role of thoracic epidural analgesia in major thoracic and abdominal surgery
Abstract
Perioperative pain increases morbidity in patients undergoing major abdominal and thoracic surgery. Thoracic epidural anaesthesia (TEA) with low concentration of local anaesthetics and opioids has been regarded as the gold standard when used in high-risk patients with comorbidities. It not only produces excellent perioperative analgesia, but also exerts beneficial effects on multiple organ systems by obtunding the surgical stress response. For skilful placement of TEA, it is essential to have a knowledge of the anatomy and complications associated with the procedure.
