Follow-up of psychiatric hospitalization egressies at the Ponta do Coral Psychosocial Care Center

This study aimed to report the contribution of strategies for monitoring and effecting the flow of care to those who had been detained in a psychiatric hospital at the Center for Psychosocial Care. In the results, 496 outpatients from psychiatric hospitalization were contacted, of whom 95 were admitted to CAPS, 22 were re-admitted, 108 attended another point in the Health Network, 89 were hospitalized for problems related to the use and abuse of psychoactive substances, 19 refused to attend CAPS II, 93 phones were not answered, 70 others. It is concluded that the effectiveness of the flow for follow-up of egresses requires innovation with mental health management plans that include this theme, also collective constructions of instruments with indicators of follow-up in the information system and innovation in the elaboration of technological resources as a digital platform with real-time access by distinguished and involved service professionals.
