In this case report we present a unique case of silent sinus syndrome in a 4 year old girl who, upon follow-up nine years later, exhibited reventilation of the maxillary sinus and correction of orbital floor collapse without surgical intervention. It is possible that her asthma treatment controlled inflammation sufficiently to allow for reventilation, or that the natural facial remodeling that occurs during development restored patency of the maxillary sinus. These findings suggest that in certain cases, surgical intervention may not be warranted for treatment of silent sinus syndrome.
