This presentation gives a report on how a novel technology, Electronic Communication Weekly (ECW), was used to help two patients who have Type 1 diabetes (T1D) as well as special needs. The first patient has autism, has had a cardiac transplant, and needs a tailored chronic care plan. The second is away at college and needs a tailored acute care plan. ECW is software that we designed; it processes data from patients’ insulin pumps and continuous glucose sensors (CGM) and generates automated weekly reports that give patients information on how to care for their T1D according to their own specific needs. This paper will show how we used ECW to provide enhanced care for two challenging patients.
