Morbidité et mortalité après adénomectomies prostatiques par voie transvésicale au CHU Kara (Togo)

Résumé: Objectif: Evaluer la morbidité et la mortalité après adénomectomie prostatique par voie transvésicale. Patients et Méthodes: Ce travail est une étude prospective, descriptive allant du 1er Novembre 2014 au 30 Avril 2016. Au cours de cette période 76 patients ont subi une adénomectomie prostatique par voie transvésicale selon la technique de Hrynstchak. Tous les patients ont fait l’objet d’une évaluation clinique portant sur le score IPSS (International Prostate Symptom Score) et le toucher rectal. Ils ont bénéficié d’un dosage quantitatif du PSA total, d’une échographie sus pubienne et d’un bilan préopératoire. Les incidents et accidents per et post opératoires ont été notés. Résultats: L’âge moyen de nos patients était de 69,3 ans avec les extrêmes de 42 ans à 98 ans. Cinquante-deux patients (68,42%) présentaient des symptômes obstructifs et irritatifs; 37 patients (48,68%) ont eu une rétention aiguë d’urine (RAU). L’échographie prostatique avait trouvé un volume prostatique qui variait de 35 à 285 cm3 avec une moyenne de 104,7 cm3. Avant l’intervention 48 patients (63,16%) étaient porteurs d’une sonde urinaire. Une cure simultanée de hernie inguinale a été faite chez 26,3% des patients. La durée moyenne de l’intervention étaient 61,5 ± 12,09 minutes. La perte sanguine moyenne après l’intervention était de 500 ± 115 ml. Quinze patients (19,74%) avaient bénéficié d’une transfusion sanguine. Le délai moyen de port d’une sonde urinaire en post opératoire était de 07 jours avec les extrêmes de 04 à 15 jours. La morbidité post opératoire se résumait en un abcès de la paroi dans 13,16% des cas, une fistule vésico-cutanée dans 7,9% des cas et une orchiépididymite dans 15,8% des cas. Nous avons noté également une incontinence urinaire dans 9,21% des cas et globalement le score d’IPSS était inférieur 07 dans 95% des patients avec un recul de 06 mois post opératoire. La mortalité a été de 2,63% des cas. Conclusion: La chirurgie ouverte pour adénome de la prostate est la plus pratiquée dans notre contrée. Cependant nous avons noté de bons résultats, mais émaillés de quelques rares complications. La chirurgie endoscopique doit être développée afin de minimiser le taux de morbidité. Abstract: Purpose: To assess morbidity and mortality after open prostatectomy for benign prostatic hyperplasia by the transvesical approach. Patients and Methods: We performed a prospective, descriptive study covering a period to November 1st, 2014 to April 30th, 2016. During this period 76 patients underwent open prostatectomy for benign prostate hypertrophy with Hrynstchak technique. All patients were evaluated clinically by using the International Prostate Symptom Score (IPSS) and by rectal examination. They have all had a check of Prostate Specific Antigen (PSA), a suprapubic ultrasonography and preoperative investigation. The incidents and accidents were noted. Résults: The mean age was 69.3 years (range to 42 – 98 years). In all, 52 patients (68.42%) presented with obstructive and irritative symptoms; 37 patients (48.68%) presented with acute urinary retention. The average volume of the prostate as measured by suprapubic ultrasonography was 104.7 cm3 (range to 35 – 285 cm3). Before operation 48 patients (63.16%) had transurethral catheterization. A simultaneous repair of inguinal hernia was done in 26.3% of cases. The average operating time was 61.5 ± 12.09 minute. The average blood loss after surgery was 500 ± 115 ml. Fifteen patients (19.74%) have received blood transfusion. The average time for transurethral catheterization in postoperative was 07 days (range to 04 – 15 days). Postoperative complications included wound infection (13.16%), vesico cutaneous fistula (7.9%) and epididymo-orchitis (15.8%). There were also urinary incontinence in 9.21% of patients and globaly IPSS scores were under 7 in 95% of patients, six months after surgery. The overall mortality rate was 2.63%. Conclusion: The open prostatectomy in our environment is still the commonest surgical option for benign prostatic hyperplasia. Therefore, we had good results with few complications. There is a need for the development of transurethral resection of prostate to minimize the rate of morbidity.
