Neurosarcoidosis is a rare variant of sarcoidosis and can affect any part of the nervous system. Cranial neuropathy is one of the most common manifestation. A 5-year-old female presented with right then left sided facial weakness, dyspnea on exertion, profound right sided hearing loss. Imaging revealed pulmonary lesions and biopsied to reveal non-caseating granulomas. MRI brain revealed bilateral internal acoustic meatal lesions that fluctuated in size with resolution after steroid treatment. Profound hearing loss was persistent, and patient was fitted with a hearing aid.
