Psoriatic arthritis is an inflammatory condition associated with psoriasis and a negative test for rheumatoid factors. The male/female incidence ratio is 1/1.04; however, vertebral involvement is more prominent in male patients. Severe, persistent cases resistant to multiple drugs can be managed with anti-tumor necrosis factor (TNF) inhibitors, which have been successfully used in the treatment of inflammatory rheumatic diseases. Presently described is the case of a patient with psoriatic arthritis who developed a paradoxical inflammatory reaction after an infusion of infliximab together with exacerbation of the disease and was successfully treated with anti-TNF inhibitors.
