Heart failure with normal ejection fraction is a condition that has the potential of deterioration during the perioperative period. In diastolic heart failure, the left ventricular systolic function is preserved but diastolic abnormalities can precipitate acute decompensation in a perioperative setting. It is important to understand and recognize abnormalities of diastolic filling of the heart for proper diagnosis, prognosis, and institution of treatment. Preoperative identification of risk factors is essential. Anaesthetic and perioperative management should be balanced and individualized.
