While sinusitis is a frequent affection in the sinuses, intracranial suppurative complication is rare and this makes difficult the diagnosis and treatment. The morbidity and mortality of intracranial complications of sinusitis have decreased significantly since the advent of antibiotics, but diseases such as subdural empyema and intracranial abscess still occur and require a prompt diagnosis and treatment and often need surgical drainage to prevent death. A 48 year old patient is presented with a subdural empyema debuting with convulsive crisis, mental confusion and focal motor deficit associated with bilateral frontal sinusitis.
