Abstract This four-part case was designed for multiple educational purposes. It may be used as a problem-based learning case with medical students; as a one-session mini-case or clinical discussion; or as the basis for role-play discussions during faculty development workshops. This case has been used successfully in US and international microtutoring workshops. International participants confirmed that the content was culturally meaningful and important. The length of the case lends itself to four 1-hour exercises.
