Treatment of pain syndrome in musculoskeletal pathology remains an urgent problem, especially in the elderly. Despite the current range of pain medications, treatment of chronic pain remains a clinical challenge for many reasons (firstly, progressive nature of the disease, secondly, the presence of comorbidity, thirdly, elderly and senile age of patients that increases the risk of side effects in such patients). In recent years, the findings of the researches suggest that neuropathic pain component is a part of pain syndrome in osteoarthritis, rheumatoid arthritis, lower back pain, although there are no data on its presence in osteoporosis and its complications. Identifying the osteoarthritis signs of neuropathic pain in patients with musculoskeletal diseases is necessary for the choice of treatment strategy and the use of targeted therapy for neuropathic pain. The paper presents the data of literature review related to neuropathic pain component in the most common musculoskeletal diseases. This part deals with conditions of occurrence, clinical features and pharmacotherapy of neuropathic pain component in osteoarthritis of the large joints, rheumatoid arthritis, lower back pain.
